I am happy to share with you the exact same Client Information Sheet that I use in my own tax business. I have attempted to make our Client Information Sheet as professional and thorough as possible, while at the same time, streamlined and easy to read and understand.

Many of our tax clients receive the Earned Income Tax Credit and Additional Child Tax Credit, among other refundable tax credits. I have spent several tax seasons fine tuning our Client Information Sheet to help ensure that we ask all questions and document all answers needed to meet the Due Diligence requirements for each client.

Our clients fill out the information sheet prior to tax preparation. We then go through and verify all information with our clients during tax preparation and make additional notes as needed. Also, with more and more tax clients requesting remote tax preparation via secure email, client portal, etc., I have made all fields fillable!
